I am trying to understand how different governments of the world
annually update the capitation rates they pay to health plans. One specific
question I have is related to the U.S experience: ¿What
methodologies are used by the different states to annually update the 
medicare and medicaid capitation rates? (for example: what information
do they use to do so? do they negotiate with health plans? how
important are budget restrictions in this process?). Many thanks for your help.
