There is a group in Washington that serves exactly the need you
describe. I just noticed that Pink posted a comment describing this
group, so I won't post this as an Answer.
The Decod program at the University of Washington School of Dentistry,
which began in 1974, trains dentists to travel in the community
delivering care at patient's homes who wouldn't otherwise be able to
receive dental care. Here are two articles on the program from
