I need a list of ALL custom cabinet (kitchen) makers in the United
States with name, address, telephone, (& email when applicable).

I spent some time looking for the list but all the lists I could find
were partial, local or belonging to one association or another.

So the only exhaustive solution I can think of to get all the cabinet
makers in the US is to use an online yellow pages service.
I tried http://www.switchboard.com/, I entered cabinet makers in the
"find a business" section and specify Alabama in the State field.
The result was 438 listed cabinet makers. (unluckily there was no way
to restrict the search to kitchen in the business field but in the
description some of them specify if they do kitchen).
If you follow this system you will have to perform the query state by
state ...

I know that this is probably not the answer you wanted to hear and I
thought that contacting a national cabinet maker association could
provide you with the list but after checking some of the association
web sites I reached a different conclusion.

In fact I also look for cabinet makers associations and I found the
following

The Kitchen Cabinet Maker Association
http://www.kcma.org/
They have a list of members at http://www.kcma.org/list_active.htm but
these are only their members and not all the cabinet makers in the US.

The Cabinet Maker Association
http://www.cabinetmakers.org/ they do not have a list of members on
line.
I thought about contacting them but once again they will have at the
most the list of their members that is most likely a subset of all the
cabinet makers in the US.

Wood Products Manufacturers Association
http://www.wpma.org/ they have a list of members at
http://www.wpma.org/inq/weblink.html but only some of them are cabinet
makers

I though a lot before posting this answer and I would have not
suggested the yellow pages solution if I thought that there was any
hope of obtaining the list any other way.
