CA Department of Social Services has an excellent search page,
allowing one to search senior care facilities by zip code, county,
city, area code and name. Updated weekly, it is easy to use and may
allow you to expand the area of your search to other nearby
communities:

My wife is a discharge planner for a major hospital, so this is
similar to the questions that she receives daily, when families have
to plan for their patients after a hospital discharge. She suggests
strongly that you call your local senior information and assistance
program to determine which programs are best for a Jewish senior. One
such program is here on the web:

There are a number of dedicated people working in this area, from
hospital discharge planners to rabbis. Several phone calls will
probably reveal resources that not even the Internet can divine. |
