A while back I found a sort of questionnaire for couples online. I
dont remember the website and I cannot seem to find anything similar.
There were multiple questions relating to your past such as childhood,
relationships etc and also questions about who you are now. The idea
was for each person in the couple to fill out the questionnaire for
the other one to read which enable you to get to know one another
better. I would really like to find this again if possible. All I
remember is that it was on a website relating to couple counselling.

There is a couples' questionnaire similar to what you describe on the
Marriage Builders website. It's called the "Personal History
Questionnaire," and is one of several questionaires designed to assist
couples in learning to know each other better, and create a better and
more meaningful relationship.
