I want to file a petition of appeal of a DUI conviction issued by
Pendot in PA without hiring an attorney.  Case was heard in Delaware
County, Media, PA.

The charge for DUI gives a alchohol level, however, the individual was
not given a breathylizer or blood test and had not refused to take
either.

I would like to know what I can do without an attorney to get the
necessary form to file this appeal and instructions for how to
complete it.  After that, I know I have to take it to the court, have
it time stamped and send it along to PENDOT.

Of course you are entitled to file anything you desire. No question.
Please be terribly careful of the court rules. The law is easy - it's
the court rules that destroy pro se plaintiff's submissions.
