My father was a chain smoker (at least a pack a day, most years two a
day) from age 18, drank coffee instead of water, and ate a very
high-fat diet for at least the last 25 years of his life.  He died at
age 62 of a heart attack.  Should I mention his health to my own
doctor, given that I believe his health issues were created by his own
lifestyle rather than genetics?

I need to clarify my father's age at death; he was 64.  My mistake. 
His initial diagnosis of heart disease was after age 55.

Just a free comment, but yes, you should mention them and leave it up
to the doctor to interpret, even if he doesn't ask.
