I am looking for a survey from a well known company conducted in at
least Nov 2001 that covers the following topic. In particular I would
like to see specific percentages for search engine use, tv, and radio.

What are the most popular ways that Internet users locate Web sites?

So confirming your requirements,
data newer than Nov 2001,
and percentages for search engine use, for TV use and radio use as 3
consumables of leisure time?
e.g. users spend 15% of their time using search engines, 45% watching
TV, 10% listening to radio?

No. I am basically looking for an current north american version of
this report (from Forrester May 2000) or a similar report or graph
detailing how Internet users find Web sites.

I could only find 1999 statistics from another source, and vague
single methods of locating websites for 2000-1
