I need to get non-identifying medical information from my sealed
adoption records in Arizona. If you can help me get the info please
let me know. I will work with you for further payment if you can
really help.  I currently live in Florida so I am not in a position to
do the work in person at the courthouse if that is needed. However, I
could travel if I had good idea of what I needed to do once I got

Non-identifying medical information is accessible provided you are 18
years old.  According to the Adoption and Medical Information
Registry, non-identifying medical records may reveal:
 
- ages at the time of the adopted person's birth  
- heritage, including their nationalities, ethnic backgrounds and race
- education (number of school years completed)  
- general appearance - height, weight, color of eyes, skin , hair  
- religion  
- talents, hobbies, special interests  
- occupations  
- health histories at the time of the adopted person's birth  
- facts and circumstances related to the adoption  
- name of the agency that arranged the adoption, if any.

Adopted persons, born in Arizona, requesting non-identifying medical
records must go through one of two steps: 1.  Get a motion from the
Juvenile County Court (where the person in question was born) to open
your private record or 2.  Hire the Confidential Intermediary Program
 
Opening up your private records will allow you to view very general
information on your birth records.  These records could vary depending
on the hospital.  They could be very basic records (such as your eye
color and heritage of your parents) or in-depth information (such as
health histories, occupation of biological parents).
 
If you’re looking to get a motion passed to open your private records,
you’ll have to find out what county your hospital of birth is located.

After locating the county, find the Juvenile County Court for the
jurisdiction in question.  A list of all Juvenile Courts are found

Call the county clerk and tell them you are adopted and want to
receive non-identifying medical information.  They’ll send you forms
to review and sign. After you return the forms a motion will be
requested to open your record.
 
Another alternative is to hire the Confidential Intermediary Program. 
Their services range from accessing non-identifying medical records to
getting information on birth parents.  An approximate fee of $250 will
be charged for accessing non-identifying records.

Lastly, after doing extensive research on the topic, it’s possible to
obtain your non-identifying records without leaving the state of
Florida.  Time spent, however, will be on mailing/faxing forms,

The adoptee has the legal right to petition the court for any
non-identifying information (kept by the county the adoption was
finalized
in, Maricopa County address available at
http://www.superiorcourt.maricopa.gov/juvenileCourt/index.asp). 
Please keep
in mind that typically the medical information available is slim to
none,
i.e "healthy baby, brown hair, brown eyes...."  Adoptees may utilize
our
services in order to obtain non-identifying information.  I would be
more
than happy to send an application to an appropriate address.

You can file a Motion to Open the adoption file with the Clerk’s Office and
the form can be obtained by fax or mail so you don't have to be present to file.
