Are you certain of the wording in your request? 

I think that the "Futures and Securities Authority" is actually the
"Securities and Futures Authority".

Does this sound reasonable to you? 

As for the "Representative Appointment Agreement" - I could find no
such document mentioned in the UK. Are you sure of that title? If what
you've given is only a "guess" at the title, could you please give
more details about what the document is about so I/we could find the
correct reference for you?
