Hi,
     My wife has done a 4.5 year course in Physiotherapy (BPT) from
India. She has applied for a license to work in the US and is
currently in the US on H4 Visa. I would like to know if she would need
to take the TOEFL test to convert her H4 visa to H1 once she gets the
license to work in the US. Also what would be the steps involved in
getting her visa converted from H4 to H1.

I understand that TOEFL is needed for visa screening which,in turn,is
needed for Green Card.But does she need visa-screening for visa
conversion from H4 to H1?

Hello,

To answer your query on TOEFL: 
TOEFL is not a requirement for H1 visa. She would not have to take
TOEFL to get her visa converted from H4 to H1.

Converting H4 to H1:
After getting license to work in the US she would need a company who
is ready to sponsor H1 visa for her. You cannot sponsor H1 on your own
(like H4 which is a dependent of H1 visa). Below link has more
