Dear Diahelp,
There are several ways to get people to actually give you their email
address of their own will, and getting their agreement to receive
promotional materials from you.
Basically, email marketing here is not different than marketing in the
"brick-and mortar" environment. You have to create some sort of
incentive that would encourage people to give their email/snail
mailing address.
This could be, for example, creating a sweepstakes/competition on your
website. One of the conditions should be to fill a questionaire, which
will include information you'd like to collect from potential
customers.
Another such promotional idea would be to give samples of your product
(or some other promotional product) away to "every 3rd person" (or
whatever) entering. After creating such an incentive, you can contact
sites that specialise in advertising freebies/competitions (search for
these terms on Google, there are thousands such sites) and let them
know of your competition.
See, for example:

You can also check for sites/newsletters that are targeting diabetes
patients, advertising your product.
