I am searching for a .pdf copy the previous version of the NASD U-5
(termination) form.  In particular, I am trying to find out what
questions 13(b)1 and 16(a) were.  The NASD changed the form in 2003, I
believe, and the older forms are no longer available.

They updated form U-5 again as of 10/2005.  (just for your knowledge)

I dont think it will be to easy to find a pdf of the form.  But find
the questions shouldnt be to tough.

Here is a 2002 version, but it does ntohave the questions you are

I cannot find an earlier one.
