Could you give me some suggestions for a restaurant near San Mateo
(within 30 minutes in medium traffic) that would be good to go with
friends and a friend's mother from Boston? I am thinking something
that has a fantastic reputation, not necessarily in Zagat's (though
that would be okay), but more a "secretly" great place - could be
Italian, Thai, Vietnamese, Chinese - but also that wouldn't cost more
than say $20-$25 each, excluding drinks.

Have you ever tried The Castaway at Coyote Point in San Mateo? It
doesn't fit any of the ethnic categories that you mention, but it's
pretty good. The visitor from Boston would probably enjoy the nice
views of the bay.
