Who made the first 256 megabit semiconductor chip?

A couple of clarification questions:
1.  do you want a memory chip -- or anything with 256M gates?
2.  should it have been commercially offered or just the first
announcement/prototype?
3.  does a ganged-memory chip qualify (four 64M memory chips bonded together)?
