I am looking for a data base with names and addresses of country
clubs, supper clubs, and city clubs in the state of Ohio.

I donŽt think that itŽll be possible to find just one database which
holds all the club information you want. So youŽll have to visit
several sites to find as many clubs as possible.
Here are the results of my research:

a)
You will find a database with listings of  country, supper and city 
clubs at
DigitalCity

You can select 
Digital City North Central Ohio
Ohio City in Digital City Fort Wayne
Digital City West Central Ohio
there.  Just type "country club" (or the others) in the search box
that comes next and youŽll get a list of the country clubs in the
selected region. The information appearing then includes the necessary
contact information,a map function and in some cases evaluations from
customers.

b)
More results can be found by using another way to find clubs in Ohio.
Visit the www.yahoo.com website.
At
( http://dir.yahoo.com/Regional/U_S__States/Ohio/ ) 
you find a list of cities in Ohio. Select the city and then "Find
Businesses" and  "Yellow Pages". Just type "country club" (or "supper
club" or "city club") and youŽll get a list of country clubs in the
selected city.

Searching the yellowpages.com site yields a list of about 271 country
clubs:
