I need an example STD test.  My girlfriend wants me to get tested, but
I dont wan't to get tested.  It can have blank names or whatever. 
Then I can put my name on it.  Just something that looks real enough
to fool a blonde.  I need it ASAP preferebly before noon of friday

Obviously you think you 'may' have an STD (or you do) and to keep that
from her is immoral, unethical and if it is a dangerous STD - illegal.
 Clearly, you don't care at all about this 'blonde' and you just want
to get your D@!K wet.  You should think twice about this - she wants
you tested for a reason.  So you really have only two options.  Say no
and see what she says.  Get the test and be honest.

You must have a lot of dirty little creepy crawlies all over your
genitals to be so afraid to get tested.  Yuck.

I'm pretty sure falsifying medical information is illegal.  You'll
probably get more STDs while in jail.

Guys, its not that.  I am clean, I made out with a girl, lightly.  I
didn't do any oral or vaginal or anal sex.  I would never risk that. 
At most, I have herpes 1, which is so common among teenagers.  I am
going to get tested on Monday, but wanted a mock form early on.  I'll
just explain to her I guess that at most, I have herpes 1 (cold sores
on mouth) in which she wont get unless I have sores.  I know mono is
possible, but there are only 2 out of every 1,000 teenagers with it. 
I'll just make sure I don't have sores before we make out or anything.

It's nice to know you'll do the right thing.  In the unlikely event
there is anything wrong, I'm sure you'll want to be treated.  And tell
"the lady" that she should get tested as well.
