Hi.  I am looking for links to national award programs designed to
recognize innovations in health care quality and delivery.

Specifically, interested in award programs that recognize projects,
groups, collaborations, initiatives, pilot programs to improve health
care quality, manage diseases / disease management, improve patient
care, improve patient safety or reduce medical errors, improve cost
effectiveness, reduce health care costs, promote consumer direction,
etc.

Typically, these award programs are administered by private groups,
associations, non-profit organizations, foundations, or universities,
although some may be administered by government agencies or consulting
firms.  The sponsors of the award programs often vary and can include
foundations, health care industry groups, pharmaceutical companies,
health plans / insurers, etc.

Basically, I am looking for multiple examples, as many as practical. 
With names of the organization, name of award program, and link to
applicable website or PDF describing the award program.
