Hello, Can someone tell me what form I need to use to add my husband
on my home's title deed. I want my home to be in both of our names. Thank you.

You need to sign a deed, tranferring the ownership from you to the two
of you, and probably you want to say after your two names, "as joint
tenants with right of survivorship." Tell us what state you live in
and perhaps we can find a sample for you to work from.

Careful on JT w/ rights of survivorship!!!! In certain states, that
is VERY dangerous. Your state may permit "Entireties Interests"
(Tenancy by the Entireties).

Doing deeds REALLY requires the practiced hand of an attorney - and it
isn't that expensive. There are all sorts of cheap forms out there -
and you end up paying an attorney on the back end, big bucks, when
some little fact isn't considered.

Be VERY careful.

It may not make any difference, but if it does, we need to know what
state you the house is in. Which US state. :)
