I can answer pretty much your whole question...except for the last three words!

There are national data available on the number of clinics and the
total number of employees.  However, I don's see anything that breaks
out employees by the size of the clinic.

On the other hand, there ARE data on sub-categories of clinics (kidney
dialysis clinics vs family planning clinics vs general clinics, for
instance).

There are also data on employee status (professional vs technical vs
clerical, etc).


Let me know if these will suffice as an answer, even in the absence of
size breakouts.
