I know the NTSB has form 6120.4 and individual airlines have their own
forms to follow when conducting an investigation but I can not finde
the airlines individual forms. I also need to speak with someone at
LinksPoint about their work at the World Trade Center
(http://www.linkspoint.com/docs/LinksPoint_FDNY_CS.pdf) but have been
unsucessful at any type of response. In particular I want to know what
categories users of their system entered data.

It's my guess that those forms are proprietary and not available to the public.
