If i were to get an abortion in New Jersey as a new jersey resident 
are their papers do i have to fill out? if yes what questions are in the 
questionnaire? How do you verify that this person is a resident of New 
Jersey?
   
  If i was a MA resident and i came to NJ as a minor to get an abortion 
performed is that illegal? How does the constitution protect my rights 
for an abortion?

l.  You don't have to lie about your residency to get an abortion. 
You may not even have to tell the clinic where you live.  Here is a
sample New Jersey Aboriton clinic's FAQ.  You will see that there is
no reason for them to have your home address (if you don't want to
give it).

2.  Minors can get an abortion without parental consent in New Jersey.
In 2000 the New Jersey Supreme Court struck down a "parental
notification" law stating that "A minor's right to control her
reproductive process is among the most fundamental rights she
possesses."

What if this person was not a new jersey resident such as pa or ma
where parental consent is necessary and came to New Jersey would their
be any problems with that?

New Jersey operate under the New Jersey Supreme Court ruling.  Clinics
may not ask for parental consent no matter where the client comes
from.
