I wasn't able to verify Mr. Levine's birth parents, which is why this
is in a comment instead of an answer, but I did find the following
information, which you may find useful as well. Most of this
information comes from Ancestry.com.

From the Florida Death Index at
http://www.ancestry.com/search/locality/dbpage.htm?t=2&c=12&co=0&y=0&dbid=5830

Name: Robert Levine
Certificate: 46123
Place: Dade
Race: W
Death Date: 12 Jun 1978
Birth Date: 01 Mar 1904

If this is in fact the Robert Levine you are looking for, the
certificate number above may enable you to get a copy of the death
certificate, for a fee I assume, from the State of Florida.

This is from the Social Security Death index at
http://www.ancestry.com/search/rectype/vital/ssdi/main.htm

ROBERT LEVINE
SSN 157-10-0069 Residence: 33319 Fort Lauderdale, Broward, FL
Born 1 Mar 1904 Last Benefit:
Died Jun 1978 Issued: NJ (Before 1951)

The Social Security Administration will provide you with a form SS-5-
the individual's Social Security Application -if you send them a
request. More information on that is at the ancestry.com or ssa.gov
websites.

I really wish I could have answered this one- the fee is a good one,
but I simply didn't have the resources to fully verify your answer.
Good luck with your search, and I hope this information proves useful
to you.
