"Oriented times four" or "Ox4" means that a psychiatric patient is
oriented in regard to person, place, time, and situation - in other
words, he knows who he is, where he is, the approximate date and/or
time of day, and the reason why he is in the hospital or institution.

I've never come across "oriented times 5." Where did you see it?
