My goal:  To provide ringtones on a website in all formats (except
proprietary formats) to anyone who has a phone that can recieve a
Ringtone via MMS.  In order to do so, i need to build a form in which the user
will select the right format ringtone to send a person, based on their
phone provider and model, and send them their ringtone as an MMS or in
other words an email with an attachment to
1231231234@their.provider.com

The form i have no problem with, but i need to know which phones use which formats.

I am looking for a compiled list of cell phones,(ideally by provider,
make and model), which specifies the ringtone format that each phone
is capable of using via MMS.

I specify via MMS because (for example) i have create and sent a .wav
file to a nextel motorola phone, which is downloaded, saved, and
played, but only as a media file in the media center.  it cannot be
set as a ringtone via MMS.  If only a list of phones and ringtone
formats can be supplied, it will suffice.
