Dear punky6-ga, I assume you mean a US social security card. If so, this page will give you instructions and supply the relevant form. http://ssa-custhelp.ssa.gov/cgi-bin/ssa.cfg/php/enduser/std_adp.php?p_faqid=251&p_created=958404483&p_sid=YLUHPn8i&p_lva=&p_sp=cF9zcmNoPSZwX3NvcnRfYnk9JnBfZ3JpZHNvcnQ9JnBfcm93X2NudD02MDMmcF9wcm9kcz0mcF9jYXRzPSZwX3B2PSZwX2N2PSZwX3NlYXJjaF90eXBlPWFuc3dlcnMuc2VhcmNoX25sJnBfcGFnZT0x&p_li=&p_topview=1 If you do not mean the US, let me known by way of clarification request. answerfinder-ga |
