Hi, I think I might have a few suggestions for you.

When people at the library I work at look for legal contracts like the
ones you mentioned, some use a book called 'The Most Valuable Business
Legal Forms You'll Ever Need'. It contains good examples of many of
the forms that you listed. It was published in 2001.

There's another more recent book (2004) that also might be able to
help you. It's called 'Ultimate Book of Business Forms'. This is a
book published by Enterpreneur's Magazine, and it contains lots of
examples of business forms. It even has a CD that has the forms on
there, so you can print out or fit the form to your business
situation.
