Question ID: 735024
what is the name of a dermal filler made from super-refined pigs
collagen? I have used it in the UK, so it must have been passed for
use here. It is called something like El---- (begins with El, I
think.) I think it's quite new. The answer must be absolutely correct
(ie no margin for error) because I want the antidote for it - ie if
there's only one pig's collagen marketed in this way in the world,
then that has to be the one. But if there are several, please say and
I will check them all out. I'll happily pay whether I'm told this is
the only one and it's called --- and made by --- or there are several.
(If there are maybe I should pay more and continue.)I'd love this done
asap. Thank you.

Dear Pearl Rivers,

I believe the filler you recall is called Evolence.

========
Evolence
========

Evolence: (technically: injectable crosslinked monomeric porcine collagen)

ColBar LifeSciences Ltd. (ColBar)

?Colbar is an Israeli Biotechnology company that was founded in 1995.
Its Evolence, is derived from the protein in porcine (pig) tendons.
The product is cross-linked using Colbar's own Glymatrix? technology.?

"EVOLENCE achieves its long-lasting effects because it is specifically
engineered to enhance stability and mimic the properties of natural
collagen found in the skin. The filler is produced via the
revolutionary Glymatrix? technology. This involves a unique and
natural cross-linking process that produces programmed bio-matrices.

EVOLENCE is produced in vitro by polymerization of monomeric porcine
collagen followed by glycation with the natural sugar ribose, which
unlike many cross-linking technologies does not require the use of
potentially toxic chemicals which limit the degree of cross-linking
achievable.

The sophisticated production process for EVOLENCE removes the most
antigenic part of the collagen molecule ? the telopeptides ? producing
a treatment that is highly immunologically compatible with human
collagen." -source: www.evolence.com

?Allergy Test: Not necessary, although those with sensitivity to
collagen products should probably receive a pre-test or not choose
this product at all. Only 10 cc/ml per year, per patient, is
recommended. (As is with bovine collagen)

Longevity: Reportedly up to 12 months

Cost: unknown

CE Mark? Yes, in October of 2004

Available in the United States? No. Evolence was launched at the 13th
EADV Congress in Florence, Italy in October of 2004. Evolence was
made viable to UK residents in 2005. However, Colbar advises that the
FDA has approved the initiation clinical studies necessary under an
investigational device exemption, or IDE.?

OTHER PORCINE FILLERS
=====================

Fibrel: (technically: injectable porcine derivative collagen)

Manufactured by Mentor Corp.

?This product was originally developed by an American dermatologist
named Sheldon Gottlieb and approved in the mid 1980's. Fibrel is
based on a porcine-derived collagen. Porcine is to pig as bovine is
to cow. Medical uses from porcine derivative (i.e. replacement
Aortas, veins, etc.) are less likely to be rejected than bovine
derivatives. However, the difficulty and pain related to Fibrel
injections, plus the lengthy preparation process keeps this product
from being offered by many practices. Since this product requires the
patient's serum, your blood must be drawn and spun in a centrifuge to
separate the clear plasma from the red blood cells. Yielding plasma
can take anywhere from 30 seconds to 1.5 minutes, depending upon the
centrifuge. Fibrel is a thicker consistency made from a 100 mg of
freeze-dried, porcine gelatin powder (similar to Gelfoam Sterile
Powder, Pharmacia & Upjohn Company), 125 mg of e-aminocaproic acid, an
anti-hemorrhagic, and mixed with .5 cc of your own serum and .5 cc
normal saline -- a local pain reliever can be mixed in as well but is
usually administered separately. Fibrel injections can be very
painful and usually require local or regional anesthetic. If bruising
is an issue, epinephrine may be administered with the Lidocaine.
You are injected with this mixture and reportedly within 90 days your
own collagen production is stimulated and takes the place of the
Fibrel. Two to three repeat injections may be required for deeper
furrows.?

Fibroquel

?Fibroquel is (technically irradiated porcine collagen mixed with
polyvinylpyrrolidone (PVP)) Developed by ASPID Farmacéuticos Division
in Mexico, this product is based on porcine (pig) collagen (type 1)
and also mixed with a vinyl polymer, polyvinylpyrrolidone.
Polyvinylpyrrolidone (PVP) is a hydrophilic (attracting water) polymer
made from free radical polymerization of vinylpyrrolidone monomer.
Common uses of PVP are in hairspray, powders used to mix with plasma
to stretch the use of plasma when in short supply, cosmetics, and also
used in drug delivery. Fibroquel is presented in vials containing
1.5, 4.0 and 15 cc of product.?
