How do I contact Equitable Life Assurance Society for the purpose of
submitting a claim for death benefits?

I am unsure as to which company you refer, and as the clarification
request emails are not currently working on Google Answers, I have
provided information on two companies which have similar names. If it
is neither of these two, please ask for a clarification.

company in the US called AXA Equitable Life Insurance (sometimes
referred to as Society).
Their instructions on claiming for Death Benefits appears below and the below link.

"Q: How do I file a claim for benefits?
A: If you wish to file a claim as the beneficiary for the loss of a
loved one, please call our toll-free Life Insurance Information Line,
1-888-855-5100. You will be asked to enter the policy number and
Personal Identification Number (PIN). It is not necessary to login,
simply speak "Operator" to be connected to a Customer Service
Representative. Having the Social Security number and name of the
Beneficiary available will help expedite the claim."
