I am looking for total number of common dental procedures by type for
the US and by state that is no older than 5 years.

For example:
Procedure            2004       2005
Root canal         25,543     43,432
Porcelain Crown    10,232      9,999           
Fillings          234,942    339,232  
Bridge              7,898      8,900
