Reliance Standard Life Insurance Company is a company that
“underwrites a variety of group coverages that are incorporated into
employee benefit packages,” including accidental death &
dismemberment, group life, long term disability, and PPO- and Fee Paid
health insurance (http://www.rsli.com/Company/company.asp ). Its
parent company is Delphi Financial Group, Inc., a publicly traded
financial services holding company.

Here’s a link for looking up PPO providers:
http://www.employeebenefitservice.com/partners/rsl.htm

Reliance’s homepage is at http://www.rsli.com and Delphi’s at
http://www.delphifin.com

The above sites show Reliance’s home office to be in Chicago.
However, most other references show Philadelphia as the main
location, as for example:
http://www.employerhealth.com/EHR_sample_pages/sp4161.htm , which
would appear to be backed up by this notice of a large gift to the
Philadelphia Museum of Art:
http://www.philamuseum.org/information/pr/990329c.shtml The company
has also sponsored a Little League team in the Cape Cod area
http://www.shrewsburylittleleague.com/ShowTeam.asp?TeamID=33

In addition, there are some other (connected) companies with very
similar names, e.g. First Reliance Standard Life Insurance Company of
New York, and Reliance Standard Life Insurance Company of Texas, which
apparently were formed to comply with individual laws of those states.

Here’s a Consumer Information Report on the company.
http://www.insurance.state.pa.us/csinfo/68381.htm

While the above shows a small percentage of complaints, there are a
number of online references to lawsuits against the company,
including:
