Most of the raw information seems to be here:

but only the formula for percent of attacks is explicitly given (kills
- errors / total attempts)

Does that meet your needs?

This might be helpful, download it and see if it meets your needs.

Also, I found this, it might be useful:

the response to "% for attack" is exactly what i am looking for. can
you please try to find the other two formulas the same way, or
possibly by workign backwards?

the links you provided did not help much. they provided info i
already have, or they just dont do what they claim. all the stat
calculator site does is take your email address, it did not follow up
or do anything it claims it does, i tryed it twice.

Good LORD!!!

For a relatively simple game, they sure have muddled up and
overcomplicated their vocabulary.

However, I think all the formula you need are in this document:

in Appendix A (around the middle of the document) "Appendix A
Division II Volleyball All-American Statistical Form".

See especially the section under "CALCULATED STATS ON REVERSE SIDE ARE

Three Cheers for the Division II Chrisitan Women!

Let me know if that does the trick for you.
