Average income for an ophthalmic surgeon (ophthalmologist).  If
possible break this number in percentiles and by number of years in
practice i.e. top 10% make  ... median is ... etc.  Also need
documentation to be a well recognized source which can be quoted in a
dispute.

I have not been able to find a percentile breakdown, but here are some
