Hi

I have a web form written with Frontpage which uses Frontpage
extensions. This is published on the internet and collects approx 40
fields of information and then emails them to me, I then put them into
a spreadsheet and do some analysis work.

I need someone to make this form dynamic using PHP with MySQL, the
following functionality will be required for the inital scope.

Price is negotiable for more complex response

1. Collect the information and in addition to the email I receive post
the results to a MySQL Database on the web.

2. Once the form has been completed the user should receive an email
confirming their input and a unique identifier that relates to teh
record they have entered.

3. A new page creating where a user can enter the unique identifier in
2 above which will bring up the record and allow them to edit the
data, when the user presses submit a new record will be created in the
database (the old or existing one remains in the database)and again
both teh user and I will receive by email a confirmation of the record
entered and another identifier that will enable them to amend the
record.

3. The site needs to be made secure to prevent inappropriate access to
the Database.

4. The form design needs to remain as at present and I can provide teh
HTML code that generates this form.

5. There needs to be an admin page where I can enter the name of the
database and the log on credentials.

I already ahve the ability to interact with the Datbase to do my analysis work

I'm happy to provide further details as required and this is likely to
elad to extra work for anyone taht's interested.

Hi, 

I believe I would be able to do this work for you in a reasonable time. 

Can you provide the following information:
1. Database table schema.
2. Form HTML.
3. Any software requirements (Can I use database abstraction with
PEAR, ADODB etc? Or should it be purely core PHP).

If you can outline the schema and requirements here, and provide a URL
with the necessary HTML code I should be able to make a start.
