I am trying to find out if there are any best-practice solutions
(compatible with IE5+/IE66+, FF1.5+)  to control printing of HTML
pages. I cannot use PDF, and do not know the length of the documents.

(How) can you do this...

1. custom footers/headers (I believe there is no HTML/CSS/DOM level
control possible until adoption of CSS3 Paged Media Candidate
Recommendation)

2. prevent the break of elements, e.g. a box with text. (I believe not
possible until wider adoption of CSS 2.1; currently only Opera seems
to support this via the page-break-inside property)

1) "Somewhat" custom headers and footers are possible using CSS. 
Sitepoint does a pretty good job of this:
http://www.sitepoint.com/print/ultimate-testing-checklist

2) There's no way to control page breaks using CSS, at least not yet.
You can certainly get it pretty close through trial and error.
