I would like a list of the most commonly prescribed medicines for US
residents aged 50 years and older.  It should include at least 50
items, in ranked order if possible, with both trade and generic names.
 E.g. #10 might be Zocor® (simvastatin) and #11 might be TOPROL-XL®
(metoprolol succinate).

This is close -- this is the top 40 or so.

Is it possible to identify the top 50 without the duplicates? 
(Atorvastatin, metoprolol, simvastatin, levothyroxine, atenolol, etc.,
are listed multiply in the list you provided.)
