which includes the borrower's name and contact information?

what is a "mortgage renewal date"?  Mortgages are for a set period of
time usually 30 years.

A Mortgage Renewal Date is the date that the loan for the mortgage
expires. Generally, mortgage loans are issued for period of 3-5 years,
and amortized over 25-30 years. Once the renewal date, or end of loan
term expires, the borrower generally refinances at a new interest
rate.
