I'm looking for the following statistics relating to the US:

1) What percentage of doctors are unfulfilled, unhappy, or bored with
their jobs?  what are the top reasons?

2)What percentage of patients feel: (I would like answers to all of
these, but understand of one of these specific answers can't be found)
   a) ignored by their doctor?  b) not understood by their doctor? c)
disrespected by their doctor?  d) that their doctor can't relate to
them or doesn't care about them?

6) How often do patients feel that they do not understand what their
doctor has told them about their illness or their plan of treatment?

7) What percentage of patients feel in control of their treatment?
