Its a Nike sphere-dry polo.
Its solid black with no embroidered Nike logo.
Has 3 solid black buttons.
It originally costed $45 or $60 (I'm sure it was only one of these prices)

Yes that is it! Also if you could please find the athletic shorts that
go with that shirt. They are the same brand (nike sphere-dry) and are
identical in color and in texture. I know this was not in the original
question, so I will add a $7.50 tip.

I think those are the shorts, but mine don't have the white accents on
the bottom of the shorts. This is not a problem, but it prevents me
from saying 100% whether or not these are the shorts. Can you send me
a more detailed picture, or possibly a description of the shorts from
the website you found it on?
