I am creating an order form in which customers will be able to choose
their product selections on the pre-printed form and subtotal on their
own.  I would like them be able to mail in their check for payment
with the correct amount including shipping and tax.  Therefore, I need
them to be able to figure out the shipping amount.  I will be shipping
the orders via UPS and USPS.  I need to find some sort of chart that I
can put on the order form that shows them what their specific shipping
charges will be so they can include it in their check payment.  I will
be shipping to various cities in the US and Canada.  Please help me
find a shipping chart for this purpose or some way that I can
accomplish this!
