I want the names of companies based in the US who provide preventive
health services to businesses.  I do not want companies that provide
direct services, such as leading seminars.  Rather, I want companies
who offer or broker an array of preventive services such as improving
the diet or physical activity or stopping smoking.  These companies
may not be the originators or creators of the preventive services, but
act as a middle man to offer these services to businesses.  The
businesses can then offer the services to their employees.
