First, the Embroidery Superstore is a commercial operation, used to doing
embroidery jobs in the hundreds and thousands of pieces. That means their
usual way of doing business is to receive original artwork from the person
hiring them, and then inputting that design, usually through a digitization
process, into their embroidery machines.
They could have probably worked from a direct scan of your ribbon bar, saved
and sent to them in one of the general image formats, such as JPEG.

That is still what you probably want to do, except with an operation used to
working on single pieces. Find someone with a color scanner, such as one of
the "all-in-one" scanner/printers out there, and ask them to scan your ribbon
bar, and save it to a JPEG file.
Then you can email it to a small shop used to doing small runs, such as Star
Enterprises.

One more thing, nautico-ga.
You might want to also send a photograph of a cap with the ribbon bar placed
where you want the embroidered version to appear, along with a note on the
expected size of the desired end embroidery.
