How can an non-attorney subpoena information from a privately held
company in another state that is critical for documenting truth for a
motion filed in family court?

Get a attorney - or at least have one explain to you how to do this.
You make the request through the court.  However, I don't know if the
out of state entity has to comply with an out-of-state court order. 
Another good reason to consult an attorney.
