How do I generate a list of all high schools in the US (ideally with
city and state, also ideally from any given year in the period 2002-3,
and ideally with references)?

Note this list is not complete.  My high school, which is a public
high school in NJ, is not listed.

try writing to government to see if you can get a list.BE PERSISTENT!! 
and dont take no for an answer!!
