I am trying to learn what the Alcoa health benefit options were for
non-unionized employees and for unionized employees in 2003 and 2004.
I need detailed information on EACH option available (such as, what
are the deductibles, co-payments and out of pocket maximums). A
minimum answer would include the information on page 5 and 8 of the
following document (which are pages 7 and 10 of the pdf):

A better answer would also include the information on pages 9 through
11 of the document (which are pages 11 through 13 of the pdf).
The problem with this document is that it does not specify what year
it applies to, and whether it applies to union or non union workers.
My ideal would be to have the equivalent of this document for each of
the 4 category (2003 union employees, 2003 non union employees, 2004
union employees, 2004 non union employees).
I could use 2002 information instead of 2003, and I could use 2005
information instead of 2004. Other years are not useful.
I will tip generously for a complete answer. I would also greatly like
information on the employee premiums (i.e what the employee has to pay
each month in order to joint the plan) for each plan in each year
(separately for union and non union employees) and will pay extra for
that; this may vary across plant locations.
