Make a table rating the 5 cheapest phone cards, as indicated in
medicalcountries.org. Indicate the cost, to call to & from, Chicago,
to & from London, Mumbai, Bangkok, Budapest, and   N.Y.  City.
Indicate how the card companies would round the time, monthly fees,
connection fees, maintenance fee, minimum usage fees, expiration date,
hidden costs, additional fees for smaller purchasers, whether you can
call from some numbers without putting in a pin number, and any
unusual contract terms. Use your creative ability to indicate what a
card user would need to notice to get the best deal, and give us your
advice, or comments. A person in a high medical cost country may need
to telephone another low cost country, for their medical needs.

Thank you for your question!  Thank you very much for reposting this
for me.  I'm sorry for the delay in response; all I can say is that
I'm a tortoise, not a hare ;)

Here are your tables.  I've found what are basically the five best
calling cards with the best deals.  For each of the countries, I then
went to each card's site and I list the link to that card or cards.
Forgive me for the tables being a bit crude, but it's the best I could
do in this space and with my lack of artistic talent.  If anything
seems to be unclear, let me know.

It is easier to get a good price to call from the US to other
countries than from other countries to the US, so the prices from
Budapest, Mumbai, et al, are not nearly as good as from the US to
those places.  Calling from those places usually means paying those
"taxes."

Calling cards can be tricky. If one has a low per-minute fee, it
probably has a connection fee and rounds to the nearest four or five
minutes instead of to one minute. Others charge more per minute but
might round to the nearest second and have no maintenance fees (fees
charged per week or per month just for not using the whole card up
yet).

Other companies charge "taxes", which are really deceitful fees,
because there are no taxes on calling card calls.  The company is
charging a fee and trying to make it seem that the government is
forcing them to implement a tax per minute, which is certainly not the
case.  I would not trust or do business with a company that charges
fake taxes on calling card calls.  This is definitely something to
watch for.  In fact, most calling cards seem to charge these taxes, so
that is why I wholeheartedly recommend Nobel.

Overall, the very best phone cards can be found at Nobel, which is a
company that I have done business with before and whose product I
trust.

US-UK: 

Features to watch out for:
nobel.com offers local access numbers for those who are calling to or
calling from a mobile phone in Britain, or from a landline with no
long distance plan. The local access numbers provide more minutes for
the same cost as just calling directly through the mobile phone would
because they don't include as many long distance charges.

VOIP:
I suggest that an even better deal than calling cards would be VOIP. 
VOIP (voice over Internet protocol) is widely available in the US from
companies such as Lingo (http://lingo.com/) and Packet 8
(http://www.packet8.net/).  I would not go with Vonage as they are
more expensive than these other companies to pay for their ubiquitous
advertising.  With my VOIP from Lingo, I can call anywhere in the
United States or western Europe and a few other countries for free. 
Other countries have a low per-minute rate without the restrictions of
calling cards.  Most people have phones in their homes, and if you
have both a landline phone and an Internet connection, I highly
suggest switching to VOIP.

If both you and the person you're talking to in the other country have
a computer with broadband Internet, you could use Skype.  With Skype,
you can make calls from your computer to a regular phone or from a
computer to a regular computer.  They actually have very competitive
rates and calling plans for making calls from a computer to a landline
phone.  The quality is very good. I highly recommend it.
