Keep in mind that not all "heads of state" are also "heads of
government" and visa-versa, particularly in parliamentary systems.
Also worthy of note is the fact there there are dictatorships,
quasi-presidential systems, coups, juntas, religious leadershhips, and
semi-presidential systems where the head of government role is
fulfilled by both the listed head of government and the head of state.
The document below addresses all of these.
As you pointed out the CIA has the information available by country,
but you may not be aware that it is also available in a single .pdf
document that you can access here. The data is current as of November
2006 making it by far the most recent and most reliable document of
it?s kind. When you say "one page" it is unlikley that you'd find them
all on one page since all of them wouldn't fit on a single (paper)
page. By one page I assume you mean "in one single place". If so, this
is a single source, printable, 114 page document with very little
extraneous information that contains "the list" you are seeking.

Please let me know what you think of this as a potential answer since
I know of no single document that contains such a comprhensive list.
