It looks like you'll have to purchase a list of all pharmacies, and
that could be somewhat expensive (but possibly worth it, depending on
your purpose). I am listing several options for you below:
Orthopedic Surgeons Mailing Lists

This page (also found by jackburton, below) lists several datasets for
sale from the Doctor's Marketing Service. The USA Pharmacies List
will cost you
$1,095 for 75,000 records (each includes name, address, and phone/fax
number).
U.S. Pharmacist
http://www.uspharmacist.com/
This trade magazine rents its subscriber list in units of 5,000 names.
The pricing depends on the file format, transmission, sorting
options, and other options. Go to the "rent list" link to get
information and a contact name for ordering. You can choose to
purchase just the subscriber areas in which you are interested or the
entire list of 100,000+ names.
I found these two links by using a variety of Google searches using
the following keywords:

list or directory
ascii or excel
If neither of the above lists work for you, you can also try
purchasing membership lists from the various associations to which
pharmacists belong. They include:

I'm sorry that you feel I haven't answered your question. You stated
that you were looking for a list in ASCII or Excel format of all US
pharmacies. To the best of my abilities I have determined (as have,
from the comments below, other researchers) that such a list is not
available free on the web, but must rather be purchased. Since the
purchase price of both lists that I found far exceed the amount you're
willing to pay, I cannot purchase them (neither would I be technically
able to attache the files to an answer, even were I able to purchase
them). I found what I felt were the best (if not only) two sources to
purchase lists in ASCII or Excel format of US pharmacies. I thought I
had answered the question - I've found you two sources of ASCII lists
of tens of thousands of US Pharmacies, with full contact information.
Please let me know where I missed the boat so that I can try to get
back on it. :)
