Hello.  How can I obtain a copy of autopsy results for a dear friend
of mine?  I do not know his family, which he wasn't close to anyway. 
Is there some address I can write to or # to call where I can pay a
fee and be mailed a copy??  He died in NJ in 1995.  Can you help?

To obtain a copy of the report  you can send a written request to:

Middlesex County Medical Examiner's Office
595 New Brunswick Avenue
Perth Amboy, NJ 08861.

Your request MUST include:
* Your name, and complete address
* The name of the deceased and date of death
* Your relationship to the deceased
* The original signature of the requesting party

Copies of records via fax or e-mail cannot be sent at this time.

The charge is $2.00 per page, payable to "The County of Middlesex".
Reports typically range from seven to ten pages.

Do not include payment with your initial request. When your request is
received, the status of the report is checked. If the report is
complete, you will receive an invoice stating the amount due. If the
report is not complete, your request will be held on file and you will
be notified once it is completed.

If you have questions about the autopsy you receive you can contact
the Chief Medical Examiner's Office at (732) 826-0331 and ask for the
pathologist who performed the autopsy.
