did nancy reagan have a cosmetic procedure which qualified as a tax deduction?

Greetings!  Although Nancy Reagan may have had cosmetic surgery and
claimed it as a tax deduction, I could not locate a solid reference to
it online.  She is a breast cancer survivor, so any cosmetic surgery
in that area would have probably been insurance deductible, but I'm
not sure about tax deductible.

I could also find no evidence that Nancy Reagon took any surguries as
a tax deduction. I think the answer to your question is: "No."

An individual's tax return (which would be the only official record
where the deduction could be taken) is not a matter of public record. 
Unless she openly admitted to it in an interview (and I don't see why
she would), I don't think the answer will be out there.
